d3dc2b9fab Merge branch 'code-mode-ui/live-parallel' into code-mode-ui/dispatch-spill
Conflict resolution: scripts/type-equiv.manifest.json takes master's new
paired-derivative format (one primary entry per pair) and re-adds this
stack's CodeDispatchLog entry in that format. zh README pairs brought
along for the dispatch-log arm (spill-policy behavior/limitations bullets,
tools limitation bullet now pointing at the shipped bounding).

04f0435cc9 refactor(tools): let the terminal marker ride the nested result
The staged-promotion machinery (concludingParents keyed by parent token
plus a pendingParentConclusions staging map) spread one fact — this call
concluded the turn — across three registry-side collections with manual
cleanup. Align it with how additionalContexts already crosses the same
boundary: concludeTurn() marks only its own execution, the marker rides
that execution's successful result (ToolExecutionFailure types
concludesTurn as never, so a policy-converted failure sheds it with the
type), and the composite that owns the nested dispatch forwards it —
Code Mode's binding does so beside its existing context forwarding.

The registry loses both parent-keyed collections and the promotion
block; the propagation decision moves to the owning boundary; the
structured-output consumer's own two-phase commit is untouched.

76d95fdc4b docs: bring the zh README pairs along for the parallel-dispatch contract
master's bilingual README pairing (new since this branch forked) covers
packages/core/tools and packages/client/runtime, whose EN sides this PR
edits. Translate the scheduling-contract sentences (bridge pool, SDK
overlap line, loop cross-reference, codeDispatches lifecycle) into the zh
sides — including the verbatim shared model-facing block — and re-record
both pairing records.

2a51ef85fb fix: close three seams the message-machine refactor left open
agent-loop lifecycle: dispose drains machine.done to true quiescence.
cancel()'s own running-to-idle transition can legitimately re-enter
through an automation listener (goal-session's idle drive runs
synchronously to its first await) and replace done with a fresh
admission after the single capture; teardown now re-cancels and
re-awaits until the slot stabilizes, so the scope never unwinds under a
live run.

tools: a nested concludeTurn() stages on its own execution and promotes
to the enclosing composite only on the call's authoritative successful
verdict. A post-execute policy that converts the nested success into an
error no longer lets a recovering composite stop the turn on a failed
terminal operation (the Code Mode structured-output shape).

goal-session: the driver owns its round durability barrier again. The
loop's persistence is eager write-behind with no turn-end flush, so the
old post-turn agent/error signal for flush failures never fires; a
settled round now sets needsCheckpoint and re-enters drive, flushing
before the next reservation and disarming on failure instead of queueing
an autonomous round on state that was never persisted.

338da9f2e0 refactor(agent): fold agentInterruptReasonOf into loop-private slot invariants
The public classifier existed to defend an exported reader against
arbitrary signals, but its only production caller is the loop reading
its own machine-private turn signal, where cancel() is the sole aborter
and always writes one frozen canonical cause. Delete the export and its
15-line structural validation: settle() states the slot invariant with
one cast, the boolean call sites ask signal.aborted directly, and the
retry veto drops entirely because a requested window already implies a
live signal (cancel() retires the window before aborting).

The abort(reason) channel and first-wins semantics are unchanged; only
the reader's publicness is gone, and with it the paranoia it required.

ffb6435a41 test(session): cover the header baseline validation and persistence round-trip
CI per-file coverage flagged the new sandboxMode/approvalPolicy branches:
invalid-type cases in both session header validation tables, and a shared
persistence-contract case pinning that both backends round-trip the
baselines verbatim and keep absent fields ABSENT (presence is the signal
the policy owners branch on).

2bc994900b fix(agent-loop): order admission publication and gate idle on committed turns
Three ordering fixes on the admission and settlement boundaries.

kick() installs the abort owner, marks the interval busy (running is
emitted before any listener can observe the claim), and installs the
pending done BEFORE publishing agent/inbox/dequeue: a dequeue listener
that cancels or disposes now finds live cancellation and quiescence
ownership instead of the previous activity's settled state, and claimed
prompt admission — including asynchronous prompt-submit hooks — sits
inside the running interval where cancel routing can reach it. The
admission-rejected path yields one microtask before continueOrIdle so
the idle transition cannot fire inside send()'s synchronous extent.

agent/idle now names only committed turns: a run that aborts or fails
before its turn/start commits exits without the notification, since
there is no durable turn/end for settlement consumers to act against.
The event's JSDoc states the narrowed contract.

c3c10820ba fix(tools): bound the shaped-append side channel; total error containment; recorded spill snapshot
Responding to ds-review-bot round 2 on #661:

- logWork is bounded: past maxParallelSubCalls pending shaped-append tasks
  the ordered commit lane holds (Promise.race drains one), so a slow spill
  backend backpressures the run instead of accumulating unbounded pending
  I/O and retained results. Tasks self-remove on settlement; run
  settlement still drains every task inside the open turn. New spill test
  drives three oversized reads against a hung backend at cap 1 and proves
  the third dispatch cannot start until a save drains.
- shapeDispatchLog's catch uses errorMessage() (total), so a thrown value
  with a throwing toString cannot escape the containment and lose the
  settle event.
- CodeDispatchLog.content documented as the RENDERED result projection
  (native tool/result vocabulary), not what the program received — the
  program gets the structured value; doc pair + type-equiv re-synced.
- New RECORDED tui-agent snapshot scenario code-mode-dispatch-spill: the
  real Loader-visible composition (worker runtime + spill-local + policy)
  drives an oversized bash sub-call end-to-end; replay proves the durable
  dispatch copy is bounded to preview + locator while the program value
  stays whole (the outer result carries just the line count).

Agent Note updated (both languages).

c53e9c90db subagent: carry inherited policy overrides in the child session header
Review fix (ds-review-bot critical #2 on #623): the first-turn event stamp
had a durability hole no turn anchoring can close — an idle SessionStart-
style injection persists a complete one-shot turn before any prompt turn
opens, so a crash in that window left a resumable-looking child with no
inherited policy, falling back to a possibly wider deployment default.

The captured overrides now ride the child's creation meta into its
immutable SessionHeader (sandboxMode/approvalPolicy, neutral strings at the
session boundary — the delegationDepth precedent), durable from the moment
the session exists: no listener ordering can starve the baseline and no
crash window can lose it. overrideOf(session) on both policy services
resolves fold(events past header.seedLength) ?? header baseline, validating
against the closed vocabulary on read; stampOverride and the prompt-submit
listener machinery are deleted. The header field rides both persistence
backends (JSONL header line; SQLite sessions columns, SCHEMA_VERSION 11 —
pre-release, no migration). pty-local reads through overrideOf so PTY
spawns see the baseline too.

Red-first: header-durability-before-any-turn test (the injection crash
window shape), baseline/seed-boundary/closed-vocabulary contract tests in
both service suites; the real-wall suite (race, veto, fork stale-seed,
grandchild) re-anchored on header assertions and green. The Agent Note's
Alternatives now records the superseded event-stamping iteration with the
review evidence; bilingual docs updated.

5e8225505c refactor(agent-loop): state slot invariants directly instead of guarding them
The coverage pass left agent.ts with eight v8 ignore annotations — well
above this repo's density elsewhere. Six guarded conditions the
surrounding invariants already exclude; expressing the invariant directly
deletes the dead arm instead of excusing it: admission/run teardown
releases the abort slot unconditionally (no writer can replace the owner
mid-flight), the request-failed branch closes its always-open step
unconditionally, the recovery finally retires the window it installed,
and the driver finally drops its dead last-resort step close (every path
already closes the step). kick()'s dequeue states the non-empty-queue
invariant as a non-null assertion. The two remaining ignores are the
assertNever exhaustiveness guard and the retry()/run() slot-race guard,
both repo-wide conventions.

single-lane driver's drive() (ordered stages, mid-flight commit drain)

f9cc62266c fix(tools): single ordered driver lane for the sub-dispatch scheduler; validate the cap
Responding to ds-review-bot round 2 on #658 (three critical findings, one
warning — all rooted in the pump/commit split racing ordered stages):

- ONE driver lane now owns every ordered stage: the start append, prepare
  (pre-execute/guards), and the head-of-line commit (post-execute, context
  deferral, settle append). start() is awaited before the next entry can
  start, so concurrent submissions can no longer run pre-execute pipelines
  concurrently; only the around-dispatch/body stage overlaps, matching the
  native loop's fillPool sequencing.
- An exclusive call's barrier now holds through its COMMIT: later starts
  wait for the exclusive pipeline (post-execute included) to finish, the
  native exclusive-group semantics.
- drainDispatches() awaits the driver run itself, so a commit already
  mid-flight when the program returns is drained before run_code closes
  the turn — the settle event and deferred contexts land inside it.
- maxParallelSubCalls is resolved and validated at construction (positive
  integer), so direct construction can no longer wedge the pool with 0.

New tests: overlapping-submission ordered-prepare, barrier-through-commit,
drain-mid-commit, cap rejection. 96 keyless snapshots replay unchanged;
Agent Note updated (both languages).

6a068ec28d test: restore 100% per-file coverage for agent-loop and the acp bridge
agent-loop: behavior tests for retry-while-busy, cancelled recovery
windows, no-facts stream failures, idle-listener preemption, rejected
driver promises under whenIdle, finish-chunk failures after step close,
presentationMeta persistence, pre-aborted and torn-down create/resume
signals, and configured-start failures over existing artifacts or after
teardown. The remaining guards that no public path can reach carry
justified v8 ignore annotations naming the invariant that starves them.

acp bridge: cover the retry-adoption path (a retry turn resolves the
prompt the failed turn deferred), the no-retry quiescence rejection, and
the admission-blocked cancelled settlement; the synchronous send-throw
catch is annotated as a future-proofing guard since the machine's send()
contains listener failures.

3b63bcbeec test: cover the dispatch-log seam's contained-failure and decline arms
CI's full-tree coverage flagged three untaken paths this PR introduced:
- shapeDispatchLog's catch (a throwing tools/code-dispatch-log listener
  must be contained — the settle event logs the unshaped content);
- the spill listener's flatten-decline arm (non-text sub-result content
  passes through unchanged);
- the generated scope-key extractor row for tools/code-dispatch-log
  (registered in the scope invariant matrix like the other tools events).

06556237f4 fix(tools): close scheduler coverage gaps; regen persistence catalog
The coverage gate flagged three untaken paths in the bridge scheduler:
- the exclusive-head inFlight re-check was dead (the shared guard above
  already returns for an exclusive head with any in-flight sibling) — removed;
- the commit-cursor undefined-dispatched break was structurally unreachable
  once entries join commitQueue only after start() ran synchronously —
  reordered the pump so the invariant holds by construction, annotated;
- the finish (final-result) commit arm and the pump re-entry guard gain a
  covering test (throwing tools/pre-execute listener) and a defensive
  annotation respectively; mid-run unregistration test renamed to match its
  actual post-result settlement path.

Also covers the direct-construction maxParallelSubCalls default (index.ts)
and commits the regenerated persistence catalog for the new dispatch pair.

af3311e1f6 test: close per-file coverage gaps opened by the message-machine refactor
Downstream packages lost the tests that exercised their agent-loop-facing
edges when the loop was rewritten. Restore 100% per-file coverage with
behavior tests through public seams: llm-retry config validation and
cancellation races, goal replay drift/staleness/teardown edges, plan-mode
disposed-flush, workspace-context empty-change commits, api-proxy
synchronous send failures, acp-snapshot spill-path extraction and refresh
write-back, ACP injection-triggered turns, cli-demo and tui inbox
lifecycle edges, and agent-loop retry/settlement/lifecycle branches.
The only source changes are narrowly-justified v8 ignore annotations on
invariant guards and one redundant-guard removal (workspace-context).

2154034a0b fix(agent-loop): let a concluding tool result beat same-step steering
concludesTurn is terminal: a completed step now carries the concluded fact
to the driver, which ends the turn immediately instead of letting steering
that arrived during the tool batch (or from an agent/stopping listener)
reopen it. The steering is not lost — it drained into the log as
steering/message before the decision point, so it feeds the next turn's
request. Also annotates the driver's unreachable invariant guards
(exhaustiveness default, abort-slot ownership) for the coverage gate.

5bf4d573f7 fix: address review — staged scheduler, start-time reclassification, catalogs
ds-review-bot findings on the parallel bridge: sub-dispatches now run
through the registry's staged scheduler view (the loop's own seam) —
ordered prepare (pre-execute/guards) at submission-ordered start time,
only the dispatch/body stage overlapping, and a head-of-line commit
cursor running post-execute, context deferral, and the settle event in
submission order (new spec pins post order + context order under
out-of-order completion). Queued dispatches reclassify via
executionMode() immediately before starting, so a registry mutation
while queued flips them exclusive (native lazy-reclassification
semantics). Config and tool catalogs regenerated; the tool-catalog
metadata now names the start/settle pair and the scheduling contract.

99a9fa8509 fix: address review — python smoke caller, contract prose, fixture header hygiene
ds-review-bot findings: the packaged Python runtime smoke's scripted
run_code call gains the required description; the ToolDefinition JSDoc
and the Code Mode foundation note (both languages, pair re-recorded) now
state both required parameters; the cordis-dynamic-toolchain fixture's
request/header line is re-compacted so the header-scrub hygiene guard
passes (my earlier patch had re-spaced it). The TUI terminal fixture was
already regenerated from keyless replay in the previous commit.

4987261d55 feat(spill): bound the durable copy of Code Mode sub-dispatch results
New tools/code-dispatch-log waterfall (run via registry.shapeDispatchLog,
contained — a throwing listener falls back to the unshaped content) lets
listeners reshape the tool/code-dispatch event's content before the
bridge appends it. dsh-spill-policy registers a second arm sharing the
model-facing arm's exact replacement pipeline (same maxInlineBytes cap,
preview + locator, within-cap invariant, best-effort fallbacks), with
artifacts labeled dispatch under the sub-call id. The program's value is
untouched; read sub-calls ARE bounded (a log copy is not model context,
and read produces the biggest logs). Resolves the tools README's
uncapped-dispatch-log Known Limitation.

8a79679489 feat(tools): live dispatch lifecycle + native-contract parallel sub-calls in Code Mode
The bridge replaces its serialization queue with a pool that reuses the
native concurrency contract: submissions classify through
registry.executionMode (fail-closed isConcurrencySafe), start strictly in
submission order, overlap up to the validated maxParallelSubCalls config
(default 10; 1 restores serial), and exclusive calls drain the pool, run
alone, and bar later calls. Each started sub-call logs a
tool/code-dispatch-start event at pool entry; the existing
tool/code-dispatch settles the pair (started ⇔ settles exactly once;
abandoned queued calls log neither). SDK prompt guidance now states the
true Promise.all contract — re-recorded across every code/both-mode
snapshot (plus the stale cordis-dynamic-toolchain fixture gaining the
required description arg).

Client: CodeSubCall widens to RunningToolCall | ToolResultNode — starts
land the running shape (rows wear the native running ring), settles
replace in place preserving start order, callTime pairs to the start
time. Fixture emits start/settle pairs; jsdom pins the running sub-row;
runtime specs pin in-place settlement and out-of-order completion.

d60dea9f55 feat(tools): run_code description param + native-parity dispatch logging + web code-mode seam
run_code gains a required bash-style description parameter: presentCall
titles the card with it and moves the program to rawInput, so every
surface gets a readable label. tool/code-dispatch now logs each
sub-call's complete content/isError (the tool/result vocabulary),
replacing the bounded resultSummary and deleting the summarize/cwd
machinery — a UI renders sub-calls through the identical path as native
results. The dsh config tree mounts the worker code runtime and reads
DSH_TOOLS_MODE (temporary seam until per-session mode selection lands).

Session format stays v0 (pre-release churn). Code-mode ACP/TUI fixtures
re-recorded; TUI presenter pin refreshed; catalogs regenerated. Keyless
web smoke pins the code-mode wire contract (tools=[run_code] + SDK
prompt section).

Integrate the Commander adapter with master's headless rework (dsh -p now boots
through AppCLIEntry — the same cordis.yml composition as dsh web, web-observable

- headless.ts: keep master's AppCLIEntry-based body but take the adapter's
  pre-parsed `task` (drop the re-added parseArgs). Old startHost path and the
  now-deleted packages/host/runtime dependency are gone.
- remove the stale packages/host/runtime dir left on disk by master's deletion
  (mirrors the earlier packages/ui/acp cleanup).

7b875b9f62 fix(agent-loop): publish turn state only after turn/start commits
A pre-commit turn/start rejection previously left the machine bricked:
lastTurn had already advanced past a turn the log never recorded, so every
later turn/start violated the session invariant's contiguity rule, and the
admitted prompt lingered in the outbox to leak into the next turn's request.

Keep admitted input on the stack (an argument to run()) until turn/start
commits, then advance lastTurn, set turnOpen, and append the prompt and its
additional contexts as user/message events inside the now-existing turn.
A rejected turn/start therefore unwinds with zero shared state to roll
back, the turn number stays reusable, and the outbox never holds input for
a turn that does not exist. This also restores the documented event order:
the prompt follows turn/start directly instead of waiting in the outbox
behind any steering carried over by cancel({keepInbox}).

2dfd8635e8 Merge branch 'master' into worktree/dsh-arg-parser
Integrate the Commander argument adapter and dsh-front-door work with master's
config-tree `dsh web` (#601: AppCLIEntry + apps/cli/cordis.yml) and the
packages/ui/acp → packages/acp/acp relocation.

- web.ts: keep master's AppCLIEntry-based boot, but take the adapter's parsed
  (host, port, dev) instead of an internal parseArgs. The adapter's host/port
  defaults (127.0.0.1/3080) match cordis.yml, so always passing them is
  behavior-equivalent to master's "undefined keeps the yml default".
- apps/cli/package.json: master's expanded config-tree dep set + commander.
- retire-readline Agent Note: point the TUI refusal proof at
  apps/cli/tests/built-bin.e2e.ts (both languages), re-record the pair.
- READMEs reconciled (demo-bin removal + master's ACP/channel rewording).

007e8fd92f refactor(cli): bail early in the arg adapter instead of returning errors as data
Address review and cut ceremony: the adapter no longer models help/version/
errors as DshInvocation members. Commander owns those under exitOverride — it
prints usage or the diagnostic and one try/catch in parseDshArgs turns the
thrown CommanderError into process.exit with the intended code. bin.ts drops its
help/version/error cases; the union is the three real modes.

Domain checks bail via command.error(print + exit 1): --prompt rejects an empty
task or a stray config/--resume, empty --resume= fails loud, and --host/--port
are validated. A repeated --resume or a flag captured as a value is Commander's
standard behavior, left alone (a bad id fails loud downstream). dsh --help
discloses web via addHelpText. Net: args.ts 185 -> 112 lines.

Also fixes review nits: built-bin e2e resolves on `close`; the /resume handoff
uses `dsh --resume=<id> -- <config>` so a config named `web` stays a positional;
and stale prose (cordis.yml comment, app-boot module doc + duplicate JSDoc,
ui/README, two feature notes, an agent-loop test name) tracks the shipped state.
Removes tui-demo's now-dead plugin-include dep and vendor/loader + app-boot
tsconfig references.
