ecb8aa5b8e Add a gated Known Limitations and Deferred Work section to every package README
Every packages/*/* README now carries a canonical '## Known Limitations and
Deferred Work' section: condensed, evidence-backed bullets for consumer-visible
gaps (unimplemented features, platform caveats, MVP cuts) and consciously
postponed work (TODO/FIXME/XXX markers, RFC deferrals still open). The ten
pre-existing ad-hoc variants ('What is NOT here (TODO)', 'Deferred',
'Limitations (MVP)', 'Known limitations (tracked TODOs)', ...) are normalized
into the canonical heading.

A new doc-sync gate, scripts/verify-readme-limitations.ts, enforces the shape:
exactly one limitations-like heading per package README, byte-equal to the
canonical h2, with at least one bullet; near-miss headings fail so variants
cannot creep back. Packages with genuinely nothing to declare (dsh-brand,
dsh-timeout, dsh-subagent-mock, dsh-app-boot) are whitelisted in the script and
must NOT carry the section; whitelist entries are validated against the scanned
package set so a rename fails loud.

Wired into the doc-sync chain (package.json) and the run-gates doc-sync leaf
set; the standing rule lands in packages/AGENTS.md and the adding-a-package
cookbook; decision record in
docs/rfc/implemented/process/2026-07-10-readme-known-limitations-gate.md
(RFC index regenerated).

Also fixes two stale '(deferred)' markers claiming dsh-compact-basic is
unimplemented (the dsh-compact seam README's package table and the seam's
module doc comment).

fc6db791f1 refactor(compact): extract the shared transcript renderer into dsh-compact
Move compact-basic's private _extractText/_blocksToText into the interface
package as renderTranscript/renderContentBlocks, so the summarize path and a
future recall read path render one span identically. Byte-identical output
vs the private helpers it replaces; compact-basic delegates.

fd1071392c docs(compact): sync the compactIfNeeded prose signature with the sessionPrefix parameter
The seam gained sessionPrefix between fullSystemPrompt and signal in
18d478bc, but the compact README member table and the compaction
core-data-structures page still showed the 3-arg form and listed only
agent/system/signal as what pre-step supplies. The generated service
catalog was already correct; only these two prose homes drifted.

765052a7d1 fix(agent-loop): compose the session prefix before pre-step; hand it to the pressure gate
ds-review-bot critical (follow-up): on the first step of a resumed or
seeded/forked instance, auto-compaction ran before runStep composed
this instance's prefix, so the gate read the PREVIOUS instance's logged
prefix from the header fold — a contributor that grew across
resume/fork (skills added, AGENTS.md grown: exactly the
environment-dependent case) could under-gate and ship an over-window
first request.

The loop now composes agent/session-prefix before the instance's first
agent/pre-step (still once per instance; runStep just reads the cache),
and agent/pre-step carries the composed prefix to its listeners.
CompactService.compactIfNeeded gains the sessionPrefix parameter;
BasicCompactService.estimatePressure gates on the handed value — the
header-fold read is gone, so the estimate is exact at every step
including a resumed/forked instance's first. Composition moving before
the boundary snapshot also means a composing listener's session append
now joins the CURRENT request (documented on the seam).

New coverage: composition precedes pre-step and the seam receives the
composed prefix; cancel and disposal landing inside the composition
window drop the step cleanly; the compact gate test hands the prefix
directly.

116e2c45a8 docs: flatten single-file catalog dirs to docs/tool-catalog.md and docs/persistence-catalog.md
A directory holding exactly one file adds a path level for nothing;
cordis-catalog/ keeps its folder because it holds two sibling pages
(events.md + services.md), matching config-catalog.md which was born flat.

docs/tool-catalog/tools.md -> docs/tool-catalog.md and
docs/persistence-catalog/log-events.md -> docs/persistence-catalog.md; their
generators' OUT paths and in-page relative links drop one directory level,
and every citation repo-wide (docs, RFCs, package READMEs, generator
headers) is updated. graph-atlas.md, config-catalog.md, and the doc graphs
regenerate with the new paths.

7539231eca compact: the summary's provenance records its call envelope
compact/summary gains { model, maxTokens? } — the envelope the
summarize call actually used, reported by the backend that made the
call: summarize() now returns { summary, model, maxTokens? } instead of
bare blocks, so an overriding backend (template or remote summarizer)
reports its own envelope honestly and compactRegion logs it. 'Which
model wrote this summary' becomes answerable from the log alone, and
the one-shot summarize request — outside the loop's header-event fold
by design — is reconstructable from log + code (the reconstructability
RFC's scope statement).

8510986909 compact: summarize is a direct one-shot llm/stream call, not a loop step
The summarization request no longer dispatches agent/request — that seam
shapes the loop's conversation requests; a hand-built one-shot's
interception surface is llm/stream, like every other direct call. The
model comes from summarizationModel falling back to the agent's own. The
turn/step parameters existed on the compact seam only to feed the
agent/request dispatch and leave compactIfNeeded/compactRegion.

Groundwork for making every conversation request a pure function of the
session log (reconstructability RFC, this branch): the seam split keeps
the loop's request path as the single thing the log must explain.

Ported from worktree-prompt-prefix-stability (PR #162) where it was
review-converged; catalog and producer/consumer graphs regenerated.

232f314c3a Add generated persistence log event catalog with freshness + completeness gates
docs/persistence-catalog/log-events.md enumerates every SessionEventMap
member — the owning dsh-session vocabulary plus the dsh-compact and
dsh-hook-protocol declaration merges — with payload, surface/log-only badge,
JSDoc prose, and declaration site. scripts/gen-persistence-catalog.ts is a
pure AST pass in the gen-cordis-catalog mold: verify-persistence-catalog
(--check) joins doc-sync, so a stale committed catalog fails pre-push and CI.

The walk enforces JSDoc completeness (every member needs description prose;
@mode is rejected as a category error — log events do not dispatch on the
cordis bus), derives the surface badge from the SurfaceEventType union with a
stale-member cross-check, and hard-errors on duplicate declarations. Payloads
render through the TypeScript printer so newline-separated multi-line type
literals still emit valid one-line fragments.

Documented the five previously JSDoc-less core events (turn/step boundaries,
tool/call), removed the two stray @mode tags on the hook/* merges, and
replaced the hand-restated event enumerations (session.md hook/* table,
compact README table, hook-protocol README bullets, session README name-list
— whose merge note had already drifted) with links to the catalog. RFC:
docs/rfc/implemented/process/2026-07-04-persistence-log-catalog.md.

a29bbe1453 Add JSDoc completeness gate for the cordis surface
gen-cordis-catalog now hard-errors (aggregated, not fail-fast) when an
event lacks description prose or a payload @param, or a public service
method lacks JSDoc, a @param per parameter, a @returns on a non-void
result, or an explicit return type annotation. The this receiver and the
trailing waterfall next are exempt on events (mode machinery owned by
@mode); a stale @param naming no real parameter errors, mirroring the
@mode contradiction check. parseJsDoc now ends prose at the first block
tag (standard JSDoc semantics), so the tags never change the rendered
catalog — only Source: line pointers moved.

Fills the ~139 gaps found across the 15 surface files, extends the spec
with negative-path fixtures for every new guard plus the exemptions,
records the decision as an implemented process RFC, and extends the
AGENTS.md typed-events bullet with the authoring rule. Runs inside
verify-cordis-catalog -> doc-sync, so CI and pre-push enforce it with
zero new wiring.

cc15ef54ef Sync remaining chars-per-token prose in overview READMEs
The convergence pass found four summary-level sites still describing
the estimator as fixed char/4: packages/README.md (twice), the compact
group and interface READMEs, and compact-basic's package.json
description. All now say chars-per-token with the charsPerToken
default, matching the authoritative package README/module doc/RFC.

1f35a4446d fix(compact): address PR 110 review findings
Honor cancellation and disposal around async pre-step setup before the loop can open a step or call the model.

Route compaction summarization through agent/request so router agents can select the model, and remove the stale model argument from agent/pre-step.

Document serial events and the approximate convergence bound, regenerate the Cordis catalog, and add regression coverage for router compaction, HMR cleanup, and assembly/pre-step interruption.

b13586ff8e docs(compact): align seam docs with the pre-step seam and record session/invariants changes (CBR-003)
Codex round 1 CBR-003: several docs still described compaction as an
`agent/request` waterfall concern, and the implemented compaction RFC
claimed "No changes to dsh-session or dsh-invariants" while the diff
changed both.

- Package READMEs / JSDoc (agent, agent-loop, system-prompt, compact,
compact-basic): compaction now lives on the serial `agent/pre-step`
seam (fired after turn/start, before step/start); the structural guard
  is tool-pairing balance (`isToolPairingBalanced`), not step-alignment;
the convergence bound is strict (`>=` rejects).
- architecture.md / core-data-structures/compaction.md: same seam +
predicate + dispatch-mode updates; regenerated cordis catalog.
- Implemented compaction RFC, updated in place to describe shipped
reality: the seam is `agent/pre-step` (@mode serial) fired before
step/start; alignment is surface tool-pairing balance; the convergence
invariant rejects `>=`; and the "no dsh-session/dsh-invariants changes"
claim is corrected — dsh-session gains the tool-pairing predicate and
  dsh-invariants drops its `start <= end` replace assertion (a positional
replace makes start > end normal).

d6da8ca29a fix(compact): decide step-alignment from surface tool-pairing, fire compaction pre-step (CBR-001)
Codex round 1 CBR-001: a head-anchored compaction checkpoint was
mis-classified by the log-position step-alignment scan, so a second
auto-compaction over a checkpoint-headed surface silently failed.

Root cause: `isStepAlignedStart/End` scanned the LOG by seq, but a
`replace` op lands a checkpoint at a high log seq whose SURFACE position
is the head — its log neighbours (the open step's assistant/message) are
not its surface neighbours, so the forward scan wrongly reported mid-step.

Fix, per the agreed direction:
- Replace the two log-position predicates with one surface-anchored
  helper `isToolPairingBalanced(nodes, events, beforeSeq)` in
  `dsh-session` (renamed step-boundary.ts → tool-pairing.ts). A cut is
balanced when no unanswered tool-call precedes it on the surface; a
region is collapsible iff both edges are balanced cuts. The open-tail
and free-node cases fall out of the same counter. It also throws on a
corrupt surface (a tool/result with no matching call).
- Move compaction off the in-step seam to a new "pre-step" seam fired
  after turn/start and before step/start, so a compaction's log-only
compact/* records and its replacement node land cleanly OUTSIDE any
step (the honest structure crash-safety relies on). Renamed the event
agent/pre-request → agent/pre-step and switched its dispatch from
  parallel → serial (listeners mutate the surface as a side effect;
serial isolates them so concurrent appends can't interleave). Extended
the catalog generator to accept @mode serial.

Regression coverage: a real-loop test driving an auto-compaction asserts
the landed checkpoint is a balanced cut on both sides; unit tests pin the
checkpoint case, the mid-step injection case, multi-call steps, and the
corrupt-surface guard. Proven red on the old log-position logic.

cec32faa4e refactor(compact): turn-agnostic retention + dedicated agent/pre-request seam
Reform the compaction blueprint so a runaway turn survives and the design
stops drifting across review rounds:

- Drop in-flight-turn protection ("layer 2"). Retention is a uniform tail→head
whole-unit walk; the only structural guard is step-alignment. A single turn
that alone exceeds the window now compacts its own early closed steps instead
of being retained verbatim (the failure mode that motivated this).
- Move auto-compaction off the agent/request waterfall onto a new awaited
agent/pre-request loop seam, fired before history derivation. Compaction
mutates the surface; the loop derives once from the result — no double-derive,
and a listener structurally cannot act on not-yet-derived messages.
- Tighten compactIfNeeded to required (session, system, model, signal).
- Enforce a single-pass convergence invariant in resolveConfig: reject configs
where summarizationMaxTokens + retainTokens exceeds the threshold, so a
  compaction can never immediately re-trigger.
- Document the crash vs recoverable failure taxonomy; core session repair stays
  compaction-agnostic (a log-only orphaned compact/start is inert).
- Wire dsh-compact-basic into examples/coding-agent and add a with-key
  compaction e2e (compaction's first real-world exercise + runaway net).
- Rewrite the RFC to encode the blueprint and move it to implemented/.

The runaway-turn snapshot is a named deferred follow-up: dsh-llm-replay cannot
yet serve the interleaved summarization model call.

aa9afcefc7 feat(compact-basic): baseline compaction backend (squashed from compact-basic)
Collapses the per-round review churn of the prior compact-basic branch into a
single clean baseline on top of compact-interface, so the upcoming retention
refactor lands as fresh, well-scoped commits rather than stacking on a history
of fixes that are being superseded.

capability-seam RFC. Wires the package into the three root tsconfigs and the
cordis catalog. A backend implementation lands separately.
